Application for Qualified Service Provider

To be eligible to apply as a QSP, you must meet the following minimum criteria;

Registered consulting business
Minimum two years consulting experience in the competency area[s] that you have selected
Note, if you are applying for more than one competency area, please ensure that you provide a reference for each.

QSP Application form: Download and complete the QSP application form. You must submit via fax [905-672-1764] or e-mail.
Background Check: CME will conduct a background check for each individual consultant. This is mandatory.

The background check is conducted by an outside verification company. A fee of $250 [non-refundable, per consultant] is required to conduct the background check. Each QSP package includes the cost of one background check and will be deducted from the QSP fee should the application be approved.

Once payment has been received and processed, you will receive an email to the online form for the background check. Please note that this is an independent company, therefore a second application is being requested. You may download a sample copy of the background check form.

You will be contacted via email when your application has been approved or declined.

3. Select the Qualifed Service Provider program level you wish to subscribe to.

Download the QSP Package (Qualified_Service_Provider.PDF) to choose your package level.

Please note - all Qualified Service Providers msut subscribe to at least the CME Basic Provider level. Companies with 2 or more consultants are not eligible for the basic level and must select the QSP Plus or Platinum level.

Upon receipt of payment for the desired level your name will then be added to the web listing as well as any other benefits associated with that level.

Note: Until your application has been accepted and payment is received in full you will not be considered a QSP for the SMART-Prosperity Now program.
