When you have all of the information ready, you can cut and paste answers in the appropriate places in the online (web) assessment application form. NOTE: All assessment applications must be submitted through the online application form. Paper copies, emails or faxes are not permitted.

Our application form has a number of detailed questions about your company, the people who work there, and about the specifics of your proposed assessement, its costs and timing, and the benefits you expect from it. The questions are detailed because we want to give you the best opportunity to provide a very clear presentation about why this assessment is necessary and how it will make a difference to your company.

Who Should Complete the Application?

Because the application contains important information about your company (for example your corporate registration number) and about your proposed assessment (including details of costs and timing), different people in your organization may have to contribute different information to complete your application. However, at this time, only one person per facility can be registered to use the online system, so it might be best if the application is actually made by a senior person at the company or by a trusted associate.

When submitting your assessment application, ensure you have all documents ready to upload with your application. All documents may be uploaded in section 4 of the online assessement cation.  Failure to attach the supporting documents will delay the review process of your application. Templates are available in the tools/templates section under Market Assessment Templates.  You may use these templates if you do not have your own formal proposals, however if using your own or one supplied by the consultant, ensure that the information provided answers the similar questions on these templates as this is the type of information required to assess your application.

For your benefit, below is a checklist of all documents to upload with your assessment application.

• Last two years of Financials - must be included
• Proposal from Qualified Service Provider.  Templates are available by assessment type under the tools/templates section.
• Note that the proposal must clearly outline what the assessment will cover, the number of days and the cost per day.
